Position Overview: The Clinical Pharmacist is responsible for identifying opportunities to improve quality and outcomes of care and managing pharmacy expenditures at both individual and population health levels. This individual will provide consultation to and review of the medication regimen of the high-risk patients identified within the CareMount Medical and CareMount ACO population. This position will augment the work of the Population Health team and reports to the Deputy Chief Medical Officer.
Essential Duties and Responsibilities (including but not limited to the following):
Supports all clinical program initiatives to improve quality of care and lower health care costs.
Prioritizes new initiatives to improve pharmaceutical management within the CareMount population; identifies and measures clinical and financial outcomes for these initiatives.
Utilizes risk stratification to identify targeted high-risk patient populations by age, hospitalizations, ED visits, comorbid diagnoses (e.g. COPD, CHF, CKD), polypharmacy, and behavioral health diagnosis, for example.
Identifies opportunities to increase generic utilization, reduce drug expenditures, and develop key clinical programs/protocols using a data-driven approach.
Develops and maintains protocols to guide prescribing therapy for select disease states, optimizing use of each payer’s formulary and generic/cost-efficient therapeutic alternatives.
Implements medication therapy management/disease state management for patient populations with high pharmacy expenses or significant opportunities for improvement.
Performs research and develops analyses of services by hospital and other organizations, both local and national, creating assessments of best practices and opportunities for collaboration.
Provides accurate, unbiased, timely and cost-effective drug information to members of the healthcare team on targeted patient population.
Coordinates educational sessions on medication therapy for CareMount team members as needed; educates prescribers regarding the use of safe, efficacious and cost-effective medication therapies for highly utilized medications; serves as a resource to suggest pharmacotherapy alternatives to enhance compliance, reduce potential errors/adverse drug events, and minimize cost burden.
Works to integrate IT into workflow to enhance communication about medications across care settings.
Carries out special projects as assigned; designs, develops and maintains analyses and reports, including graphic presentation and dissemination.
Supports and participates in other duties as required to meet workload demands.
Qualifications and Requirements:
Current/eligible for licensure in the State of New York
Doctor of Pharmacy (Pharm.D.) degree
Board certification preferred
ASHP-accredited PGY-1 Pharmacy Practice Residency preferred
Experience in a physician practice, ambulatory care, or managed care setting preferred
Experience working with Medicare Part D preferred
Must possess strong assessment skills as well as the ability to work with health literacy issues in the population; must be able to communicate effectively with physicians and other professionals; must be skilled in critical appraisal of medical literature; must be capable of independently reaching therapeutic decisions in patient care and communicating them to physicians and other involved health professionals.
