As an Infectious Disease Clinical Pharmacist Specialist you will be responsible for the provision of safe, effective, appropriate and cost-efficient antimicrobial therapy for the health care system, including the adult, pediatric, inpatient and ambulatory patient population.
Principal Duties and Responsibilities:
Responsible for compliance with Joint Commission and other regulatory standards for antimicrobial stewardship
Adhere to the Centers for Disease Control (CDC) Core Elements of Hospital and Outpatient Antimicrobial Stewardship Programs
Co-chair the system level Antimicrobial Stewardship Program with collaboration amongst ChristianaCare services lines
Develop, implement, and maintain guidelines, policies, and procedures that promote and improve patient outcomes
Develop and maintain order sets and clinical decision support within CPOE to ensure appropriate antimicrobial use
Provide direct patient care activities through prospective audit and feedback by designing, recommending, monitoring and evaluating patient specific antimicrobial pharmacotherapy
Develop and maintain the antimicrobial formulary, including the oversight of restricted antimicrobial agents
Provide accurate, unbiased, timely and cost-effective drug information to medical, pharmacy, nursing and other allied health providers
Participate in identifying and submitting events via Report2Learn (R2L) system and providing follow-up assessment.
Report to Clinical Value Council, P&T committee, and Infection Prevention committee
Represent the Antimicrobial Stewardship Program and Department of Pharmacy Services on appropriate committees, as needed
Provide caregiver education about antimicrobial resistance and antimicrobial stewardship best practices.
Review antimicrobial resistance patterns in collaboration with Microbiology Lab and Infection Prevention
Develop platforms to collect antimicrobial utilization data in collaboration with information technology colleagues
Collect and analyze antimicrobial utilization data
Report antimicrobial utilization data to National Healthcare Safety Network (NHSN)
Collect and analyze antimicrobial stewardship intervention data
Precept PGY1 Pharmacy Practice, PGY2 Critical Care, PGY2 Ambulatory residents, and pharmacy students as part of the Infectious Diseases Consult/ASP Rotation
Support and assist interdepartmental projects (e.g. residency research projects, MUE, P&T monographs, newsletters, performance improvement activities, etc.)
Serve as a member of pharmacy resident committees (e.g. steer, residency research committee) as needed
Coordinate, facilitate, and promote education, training, competency assessment of pharmacists and pharmacy residents for dosing by pharmacy programs
Education and experience requirements:
Pharm.D. and ASHP-accredited Pharmacy Practice Residency required
PGY2 Infectious Diseases Residency required
Board Certification preferred
Licensed as a pharmacist in the State of Delaware or eligible for reciprocity.
