ChristianaCare's Supportive and Palliative Care Team is seeking a Nurse Navigator to support and care for our patients in our Outpatient Program. Finding the successful candidate will be key to delivery of patient- and family- centered care that optimizes quality of life for seriously ill patients and their families through this nurse's leadership, organization, and communication skills. In this role ~

• Act as a primary nurse contact for the Outpatient Services of the Supportive and Palliative Care team (SPC).
• Willingness to take a strong leadership role to achieve program goals in outpatient palliative care.
• Improve the continuum of care by optimizing the flow and tracking of outpatients seen by our teams in Community, Acute Care setting and the Helen F Graham Cancer Center.
• Provides triage and screening of referrals to ensure appropriateness of consults and manages entries into referral management database.
• Support and advocate transformation of care for patients with serious illness through educational programs.
• Support workflow optimization as a champion for continuous improvement to achieve patient satisfaction of the outpatient palliative experience.

Requirements:

• BSN required.
• Delaware RN license.
• Three or more years of clinical experience in acute or outpatient care.
• Clinical experience in team based care, chronic disease management, home health, hospice and/or palliative care preferred.
• Proficiency in EMR systems: Cerner Ambulatory, Powerchart, Soarian preferred.
• Palliative Care Certification preferred, or to be obtained within 2 years of joining the team.
