In collaboration with the physician, the Ambulatory Care Nurse identifies at risk patient populations who require care management services. You will provide bio-psychosocial assessments to identify patient needs and to assist in the development of the plan of care. You will also serve as a patient/family advocate and act as a communication link between the physician, patient, family, outpatient and inpatient health care team, as well as community resources, to reduce redundancy and fragmentation of care. Here at WakeMed, you are more than an employee – you are family. We understand that our responsibility to the community begins with our employees.
