The Licensed Practical Nurse provides direct support to the PACT team in the Ambulatory Care Outpatient Clinic or Primary Care Telehealth Outpatient Clinic. The incumbent is accountable for greeting the Veterans, using different modalities of gathering Veteran data such as observation, vital signs and recording information the Veteran provides to the LPN; communicating this data to the RN and/or provider and acting as directed.
