| Women's Health, Clinical Pharmacy Specialist (CPS) will serve women veterans across the Gulf Coast Veterans Health Care System (GCVHCS) and those receiving care at GCVHCS Community Based Outpatient Clinics (CBOCs). The Women's Health CPS is a staff member of Clinical Pharmacy Services Section, Pharmacy Service, and reports directly to the Associate Chief, Clinical Pharmacy Services and functions as a team member on the facility's Patient-Aligned Care Team (PACT). Through clinical assessment, the Women's Health CPS relates patient responses to medication therapy, communicates and documents those findings, makes recommendations to appropriate individuals and implements and monitors pharmacotherapeutic care plans for female gender specific conditions including maternity. The Women's Health CPS will work collaboratively and cooperatively with the Women Veterans Program Manager (WVPM), with GCVHCS leaders, clinicians, administrative personnel, support staff, and other strategic partners to ensure that the needs of women Veterans are met. Duties include, but are not limited to: Provided direct patient care services through individual visits, group visits, telephone visits, video conference visits, chart review, and use of secure messaging. Conducts medication therapy management clinics and group forums (e.g. diabetes, smoking cessation, gout, pain, dyslipidemia, etc.). Provides drug information, observations on patient response to therapy, and develops appropriate treatment plans regarding treatment alternatives or additional interventions to maximize patient care outcomes taking into consideration choice of therapy, safety, efficacy, and pharmacoeconomic. Evaluates appropriateness of pharmacologic therapy at clinic visits and as the level of care changes. Participates in the development of guidelines, audit systems, and data gathering efforts of the Pharmacoeconomic and Clinical Pharmacy Section in support of quality assurance monitoring and plays a role in budget execution. Maintains workflow patterns that assure timely delivery of outpatient ambulatory care pharmacy services, ensures accurate dispensing and record keeping documentation, and oversees technical and nonprofessional staff in the outpatient pharmacy setting. Dispenses controlled substances according to current policies, procedures, and standards established by DEA, FDA, VA, and Joint Commission. Assists in the training of pharmacy employees, pharmacy residents and Pharm.D. students as assigned to the Women's Health CPS in coordination with the Clinical Program Manager.
