The Float Nurse Coordinator will help cover a variety of surgery/medicine nurse coordinators when they are on leave, FMLA or retire from their positions. Float Nurse Coordinator will also have the opportunity to choose to reside in a vacant nurse coordinator position they are covering, or to train another nurse to the open role. Paid Time Off: Annual leave at the rate of eight (8) hours per biweekly pay period; Sick leave at the rate of four (4) hours per biweekly pay period; 10 paid Federal holidays per year. The Float Nurse Coordinator will help cover the medical or surgical specialties to be the liaison between Ambulatory Care practitioners and disciplines at VA Puget Sound. Duties include but are not limited to: Coordinates patient care related to acute or chronic medical conditions. Works collaboratively with patients, their families and health care providers to ensure optimal care for patients throughout their surgeries/medical visits. Provides multidisciplinary approach to patient's needs, working closely with clinicians and other departments. Assist in organizing, scheduling and supporting clinics. Main point of contact for veterans. Performs telephone, MyHealthyVet and other triage of patients. Provides tracking and reintegration of patients referred for outside care via Community Care.
